Elm Tree Having Issues

The elm in my backyard is having it's leaves get splotchy brown patches and there is a spot on the trunk that is always wet with brown goopy stuff running down. At the top of the wet area there is a hole that has small worms in it. I'm not sure if the worms are there because of it or the cause of it. Anyone know what is happening to this tree?

Bleeding area is bacterial wetwood/slimeflux.

Leaf blotches are elm leafminer or elm flea weevil.
