To me, looks like the primary issue here is 1)planting too deeply...

Other additional issues... no proper mulching, could have some root girdlers, cankers..and maybe weedeater damage gone worse...canker?


Anyway, I think the too deep of planting has caught-up and is the primary culprit to the visible crown issues, any other ideas on this tree?
But maybe Im amiss...

Treatment- They should carefully "break-up"/dig out the soil down to the flare, out within the drip line, trying not to damage fibrous roots....to aeriate and check out the roots for girdlers, and prune any girdlers...mulch properly.

Is there any noticeable differences that would take place in a crown, caused from root rot verses a girdling root?
 
Symptoms of root rot can appear acute (wilting quickly) because the tree will show after a significant portion of the root system has be infected.
Girdling issues are more chronic, longer slower decline of health.
