Loose climbing and rigging equipment was cause of helicopter crash killing four.
Loose bag of carabiners and climbing rigging equipment on outboard carry containers on a Hydro One helicopter flew off striking the tail rotor blade and sending copter crashing to the ground near Tweed, Ontario.
All four crew members were killed.
 
No excuse for that one. Pilot should have inspected the straps or cargo netting (if they even used any) before take off.

That was my thought exactly. Pilot needed to inspect the craft before take off.
You can never be too safe with loads on/in vehicles.
Too many times I've seen careless loading on the road or on water.
Open tail gates, loose and open gas cans, untied loads in pick-ups, etc.
