Maple ID and Prognosis

Gang,

Two part question here from a newb...1) Is this a Norway Maple or ? 2) Any ideas what this species of tree might be prone to that would kill it off like this? Any help is appreciated! I'm trying to decide if this is something that can be treated and restored or if I've found my newest soft wood burn victim.

Maybe verticillium.

There will be black streaks in the wood when cut.

Your tree is a goner.
