Its july now. Still has green leaves dropping. Many branches leafed out and suddenly died leaving the dead leaves this may-june.
Customer likes the tree and it is young.

I see borer holes in the bark. Just dont know what kind. Petiole? This is west central missouri right on ks line.

Something sawed this leaf off very smooth but I am sure that's a separate issue.

Looking for best treatment option for the borer issue.

The borer holes look like they are possibly Yellow Belly Sapsucker holes.
The tip die back is definitely more of a concern. You said it’s a relatively young tree. How’s the planting depth? Is there a nice flair or does it look like a pole sticking out of the ground?
If it’s not a root issue it could be verticillium wilt.

It has a good root flare.
Its probably 8 years old if I had to guess hard to tell. I was assuming the die back was from xylem loss
