Looking close at the pictures, the chain is on the side of the sprocket teeth and not centered on the teeth. yes, the sprocket is shot.

Time for a new sprocket for sure.

I would definitely replace the bar tip. However you should also clean and inspect the bar grooves closely. Any chips or cracks and it should also be replaced. File off any burrs, check the sides for flatness against a flat surface, you can hammer back any imperfections. Finally check that the groove is not too shallow. Does your new chain bottom out and feel loose at all. If so bin the whole bar and replace the lot. Bars/sprockets and chains are wear items and are designed to wear together and be replaced together.

At the very least- the chain needs reversed to the correct orientation- the sprocket tip is poked and needs replaced- the bar needs at the very least dressed- but in reality seeing as how that looks to be a new chain, go buy a new bar and a new drive sprocket.
Grease tip both sides every 6 hours when you do.

Bar may be shot but that tip and sprocket most definitely are.
Tip replace isn't hard but you need the proper tools to do it.
If you don't have the tools just replace the whole bar. Keep the old and replace that tip once you know how to do it.
Post in the "want to buy" section for a good used bar...or, just get one locally to you.
