Yoshiko Cherry Won't Blossom

I have a 10 year old Yoshiko Cherry Blossom that hasn't blossomed properly in 7 years. Most of time it will only produce very few blossoms, and some years none at all. Any ideas why, and what I need to do?
 
My inclination is some sort of abiotic disorder or combination of them.

Pictures of trunk, root flair, stems, and buds may help.

Some species are susceptible to flower buds being lost to freezes as an example of potential problems.

Here are some pics of the tree. We did have a cold winter this year, but it other Cherry trees are blossoming in the area.

I fertilized the tree with some tree fertilizing stakes from Lowes two years ago, but I'm not sure if that causes more harm than good.
