Do we know if the hostage taker fired the first shot, at people trying to escape, at which point there was only one way it was going to play out, or whether the AOS (or whoever they were) had already decided it was not ever going to end well and they seized a chance to take him out as he moved hostages to another part of the building, all but conceding that risking a few lives is the best of bad options?
