I went to the regional meeting here (PNW chapter) the last two or three years, I havent attended their training day, yet. two reports in for failures so far.
 
I've attended as well, but have been lax at filing reports. I should have sent in 20 or 30.....
 
when a tree in whole or in part fails. ie, falls over, drops a limb, breaks mid trunk etc.
 
uhm, no, I meant that I have sent a report in for a tree that failed near here. The point of the database is to try to establish a body of information regarding tree failures etc. so industry people with some basic training are invited to sign up and send in reports of failures they witness. There are actual training seminars for this as well, to make sure they dont get reports like " poplar fell over in wind " , there is a long checklist of information that is asked for regarding the failure.
