Pin Oak disease

I have a Pin Oak Tree that I purchased about three years ago. The spring after I had it professionally planted, the bark began to turn black and became very brittle and is falling off. I also have white 'fungus' growing on the base of the tree. The branches seem to be dying one by one. I trim the dead ones, and am sure the tree will be dead soon. Does anyone have any idea what disease this is and how I can combat it? I live in Southern Indiana....
