So, in your opinion, was this a defective parts? Or, was it a result of the graby chain, or ???

If the initial crack wasn't over 180 degrees I would say it was caused by an over load. Since its almost all the way around it was opened up slowly over time. There could have been a thread lapse from rolling in the threads that caused it to fatigue early or it just could have been plain worn out.

...i think this 026 has same crank end as a 660 ,10 mm bearing,12 mm clutch threads

I'm thinking design error. Whatever the crank thread size at the clutch carrier on the 660, there's evidently not enough of an engineering safety factor in the crank to resist the loads that the 660 can experience in the field.
