My crew spent all morning hand-pulling it out of a big oak's rootzone. Then we fertilized and watered. When it comes back from the roots it'll be time to spray the new growth with glyphosphate. When the leaves are new, control is possible, but not total the first time.

On a big tupelo I pulled it out of the crown but left it on the trunk. Every 3-5 years I'll be back to do it again. It's nice stuff but hard to keep contained.
 
If the stems are large enough, you could use sodium chlorate crystals. Cut the ivy so there are stumps 6 inches above the ground. Then apply the or crystals into the grooves. If there are animals or people around it, put a cap on top of the sodium chlorate so that it cant be eaten. Any translocated/systemic herbicide should do the job.
