Husqvarna 351 chain tension issues

Hi I have a Husky 351 and I've been having chain tension issues. The chain will go very tight and unable to turn so I'll adjust the screw not even a quarter of a turn and shortly after the chain will be much too loose.

I keep the oil port clean so the chain is getting oiled, any suggestions on what could be giving me these issues?

Chain tensioner? Clutch? Sprocket?

Any help would be appreciated, I spend more time adjusting my chain than cutting wood.

Check drive sprocket for wear. Replace it if wear is over about .020"
