Husqvarna 390XP recoil

The rope has come out on the inside twice. First time, it took out the flywheel, debris cover and the coil. Second time just debris cover. Why would the rope unwind inside the cover?

Defect in the plastic of the pulley-rotor where the knot inserts...? A hairline crack, maybe expanding with heat?

Have yet to have a knot slip out so I'd have to think more on it.

Broken spring possably, badly worn pully, or not put together right to begin with.

Spring is good. Probably gonna replace the pulley.
