My father has a 45 year old maple tree that he's afraid he's going to
loose. It isn't leafing out, and one planted at the same time, is doing
well. The tree has a butt of 20 ft.(?) 2 in. Thank you in advance for
any information.

I'm not sure as to what you are asking, but if a deciduous tree has not leafed out when every other tree has then it sounds dead to me.

Has there been any construction around the tree in the past 10 years?

Was there any notice of decline in past years? Usually a tree that size does not die over night. Maples are susceptable to Verticillium Wilt. This affects the trees ability to absorb vital nutrients, and water. If this was the case you would have noticed branches dying turnig completely brown and brittle.
The tree should be examined on site by a proffesional to correctly diagnos your problem.
