He looked to be a heck of a pilot. They say there are old pilots, and bold pilots, but no old bold pilots. I think you have to be pretty bold to fly timber.
 
My only experience in aviation is medical but my theory would lean heavily towards the turn line breaking and recoiling up into the rotor head causing its separation. Occupants very rarely survive rotorcraft crashes. All aeromedical providers know at least several colleagues that have died in crashes. Thoughts for all those affected.
